Support pathways
Action now. Outcomes later.
Founded and operated by veterans, we assist homeless and at-risk veterans for up to one year on the road to long-term stability.
Personalized support
One-on-one, from day one
One-on-One Assessments
Coordinated case management with personalized guidance and connection to community resources that fit each veteran's situation.
VA Services Assistance
Hands-on help recovering state IDs, Social Security cards, medical records, and DD-214 forms through accredited providers.
Live-In Peer Support
Peer-to-peer support from experienced residents reinforces structure and accountability — vets holding vets up.

Health & wellness
Whole-person care
We connect veterans with healthcare access and mental-health care, and — always optional and veteran-directed — referrals to chaplains and faith-based organizations for those who want them.
Housing & recovery
Structure that holds
Our Pathfinder Houses are structured transitional housing — 6–12 bed homes, not clinical treatment. Every property is a substance-free environment with a strict no-alcohol policy, on-site house monitors, daily structure, and medication oversight.
We coordinate with community-based recovery programs, detox, and treatment providers, and the house itself runs on 12-step-friendly, peer-supported rhythm: chores, house meetings, and honest engagement.

Long-term stability
The road home ends at a front door
The goal is independence: permanent-housing transition programs, employment and education guidance, family reunification, and up to twelve months of personalized assistance so the next chapter sticks.
